It isn't trials, as I have run cloudrest with 6 people and 6 companions - hint - its not as fun as it sounds lol Our guild events, especially fishing night can have 6 players 6 companions in a group without issue. This bug was fixed at least with overland and public dungeons. So with this change, a group of four people can have four companions This means a group of 2 players can each have a Companion, a group of 3 players can only have 1 total Companion and groups of 4 or more will not be able to have any Companions in these areas. You are not currently able to summon Companions into groups that have 4 or more players + Companions present while in the overland or in public dungeons. I looked it up and found this though, from old patch notes apparently: That or instanced quest areas if you happened to be questing with 4 other people in the same group. I wasn't aware that there was a limit to 2 companions in such an instance, so I'm wondering is this means there can be 4 companions at once in a trial and 8 player characters. I'm assuming that's referring to trials, since you can only have 4 people in a dungeon.
